    Sorry folks!

    Somehow this link got distributed in a way that was unanticipated. The incentive was only intended for the first 100 respondants of two particular diabetes forums online, and the close survey mechanism did not work properly, my apologies for any confusion! So if you are in that group of the first 100 respondants, you will receive the incentive, beyond that, we will ask if you are interested in being on our list for future surveys — we regularly have opportunities!
